Role reputed company reputed company is seeking a full-time, reputed company Assistant to help reputed company high-reputed company, member-centered care. In this role, you’ll play a key part in improving reputed company reputed company, supporting care coordination, and enhancing the experience for both members and their caregivers. This position is ideal for a compassionate MA who is passionate about improving the health and reputed company-being of older adults through personalized, holistic support. • Review Community Health Worker (CHW) appointment documentation for reputed company and completeness, escalating issues to the reputed company case manager as needed. • Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers. • Mail and/or email reputed company care plans and educational materials as directed by case managers. • Accurately update and maintain reputed company records reputed company the electronic health record (EHR) reputed company. • Protect reputed company confidentiality and ensure compliance with reputed company HIPAA and organizational policies.
